Ordered a large, sent extra large. Said they run small not really. Insist on your size. The side straps can be let out 2 inches. As the design is neither a pouch or sleeve/sling support there is gapping between the material and the boys. Sometimes one of the boys is pinched between the gap, usually when sitting. The thong acts more like panties. Over the thongs are OK, but a more snug pouch that cups/supports the boys would offer a more desirable design.
